Two different sets of documents apply here: the ones your practice signs with Greenbar to use Tahlk with patients, and the ones that govern this website itself. Both are listed below — this page is an index, not the documents themselves.
For your practice
Between your practice and Greenbar Systems, covering Greenbar's handling of PHI as your Business Associate — including the managed proxy to Anthropic. See Compliance for how this fits together.
Governs your practice's use of the Tahlk software itself — separate from the BAA, which governs data handling.
A template Greenbar provides so your practice can tell your own patients, in plain language, how their information moves through Tahlk. Your practice adapts and issues it — it isn't a notice from Greenbar to your patients directly, and it doesn't replace your own counsel's review.

These are separate from the practice agreements above — they apply to visiting this site, not to using the Tahlk application or handling patient data.

This page is an index of what exists and who it's between — it is deliberately not a substitute for reading the actual documents, and it isn't legal advice. The BAA, EULA, and Patient Privacy Notice Template are real documents handled through onboarding and your Greenbar representative; the website's own Terms of Use and Privacy Policy are drafted separately and published here once finalized. If your practice has specific requirements, raise them before you sign.
